IN THE FORM PLEASE INCLUDE THE FOLLOWING INFORMATION:
• A detailed description on the tattoo/s you’d like.
• Area of body you’d like tattooed (e.g. right inner bicep, left shoulder blade, full back piece, back of right thigh etc).
• Size of tattoo (in either mm, cm or inches please)!
• Style of tattoo (black, black and grey, dot shade, line shade or colour. Please note I specialise in black and grey and dotshade/fine line and line shading so colour is very limited.

• Reference images. You can attach up to 10 image references to describe and give examples of the type of tattoo you are after. They can be from my previous work, drawings, sketches, images online, artwork etc. They can also be from other artists/tattoo artists work but please be aware that I will NEVER copy a tattoo or artwork from another artist they will be used as reference ONLY.

• Please attach photos of the area to be tattooed IF you have any of the following:
- scarring
- other tattoos
- any skin conditions
- moles, birthmarks etc

• Please make me aware at the consultation stage if you have any medical conditions, blood conditions, medications or allergies that you think may interfere with the tattooing process.
